Job Responsibilities:

1. Reliably assists with check-in of clients. Reviews, updates and maintains patient records.

2. Consistently completes vital signs on all clients and enters into the electronic health record.

3. Consistently prepares treatment plans and consent forms and explains to patient, and obtains patient signature.

4. Successfully facilitates patient compliance with psychiatry goals/objectives.

5. Appropriately monitors physical health status and adherence to treatment recommendations.

6. Accurately assists with scheduling appointments as necessary.

7. Regularly completes telephone triage, retrieves messages, and returns phone calls within 24 hours.

8. Routinely obtains insurance authorizations for patient medication and ensures completion of processes and communication with patient acting as back-up for phone nurse or prior authorization specialist. 

9. Professionally interacts and coordinates with pharmacies, hospitals, community physicians, and laboratories.

10. Professionally and respectfully conducts his/her self in a manner defined by SalusCare Code of Ethical Conduct Policy as it pertains to providers, managers, co-workers and patients.

11. Effectively provides training, education, and support to persons with substance abuse and co-occurring disorders in the areas of: Medication Assisted Treatment for substance abuse treatment, medication side effects, treatment compliance, and psychiatric and primary care service coordination. 

12. Reliably provides medication information and obtains consents as necessary.

13. Accurately administers injections as prescribed by Physician or APRN and documents appropriately in Electronic Medical Record.  

14. Effectively and efficiently supports multiple providers throughout the day providing patient work-up, monitoring his/her schedule to maximize patient volume and assuring all available appointment times are filled. 

15. Routinely works with support staff to manage walk-in list at all locations, filling provider schedule for maximum efficiency and productivity.

16. Reliably reviews patient record with Physicians or APRN when necessary.

17. Consistently maintains appropriate customer service relationship and professional boundaries at all times.

18. Consistently places customer services in a priority position when working with patients ensuring their needs are fully met and the patient feels cared for while they are visiting SalusCare.

19. Routinely checks medications in med room for expiration date and appropriately discards and notes discard of outdated medications.

20. Consistently attends regularly scheduled department meetings as requested by supervisor.

21. Regularly attends other agency meetings as requested by the supervisor.

22. Effectively demonstrates a sound understanding of and commitment to complying with CARF standards for the medication assisted treatment (MAT) program.

23. Consistently displays cultural competence and sensitivity and ensures services provided (evaluations, treatment plans, discharge planning) incorporate client-specific cultural values.

24. Efficaciously identifies and targets care gaps and recommends and encourages the attainment of needed additional services.

25. Reliably works shifts consistent with the PRN agreement.

26. Promptly attains competence with Electronic health record software and uses it effectively within required time frame.

27. Routinely maintains active participation in at least one Committee or performance improvement team and attends that and other meetings as assigned or necessary.

28. Successfully completes chart peer reviews on a monthly basis and follows up on any deficiencies noted.

29. Unfailingly complies with rules/regulations/requirements related to: clinical documentation, all other documentation such as Outcomes, Demographics, productivity, Medicaid, AHCA, County Regulations, 65E-15 and 65D-30.

30. Consistently demonstrates and models attitudes and knowledge consistent with co-occurring best-practice standards and ensures staff are trained and practice accordingly.

31. Reliably operates as a back-up to alternate department(s) when requested.

32. Routinely maintains compliance with staff development training. Performs related duties as assigned by supervisors.

33. Consistently meets CEU requirements for licensure/certification. 

34. Continuously demonstrates good attendance and reliability by doing the following: consistently reports to work on time; reliable work performance; no abuse of leave/PTO policy; adequately plans leave and makes every effort to cover responsibilities in exigencies and coordinates training/workshops/meetings with supervisor as necessary. 

35. Successfully demonstrates an understanding and respect for cultural competence and sensitivity when interacting with employees, consumers of SalusCare services, visitors, vendors and other stakeholders.

36. Successfully performs other related duties as assigned by supervisor.

Job Qualifications:
  • Requires an L.P.N. certificate in nursing
  • Valid Florida Nursing license is required.   
  • (1) One year of experience in health, human services field is preferred.
  • Must be able to attain competency with company electronic health record and use it effectively within required time frames.
  • Requires basic clerical skills including but not limited to writing, taking accurate phone messages, working with multiline phone system, copying, faxing, shredding, basic computer literacy.
