Peer Specialist/ Outreach Worker

Job Responsibilities:
1. Regularly utilizes principles of recovery to assist patients in regaining control over their lives.

2. Consistently prepares and delivers individual recovery related activities to individuals with substance abuse disorders or co-occurring mental health disorders.

3. Routinely work with the local hospital system to educate patients about MAT services, SalusCare programming, and engage individuals into recovery. 

4. Successfully serves as an advocate, mentor, or facilitator for resolution of issues that a patient is unable to resolve on her/his own.

5. Efficiently advocates on behalf of the consumer in order to maintain needed resources; advocates in a manner that promotes independence by encouraging and teaching consumers to advocate for themselves.

6. Effectively conducts outreach to potential patients and establishes community partnerships to facilitate a streamlined referral process.

7. Routinely demonstrates outreach efforts to local community resources including hospitals, doctor’s offices and community service providers. 

8. Promptly connects potential patients with the screening process to determine appropriate level of care.

9. Consistently prepares and delivers individual recovery related activities to individuals with substance abuse disorders or co-occurring mental health disorders.

10. Consults with other treatment providers on a regular basis in order to monitor services and communicate needs of consumer. 

11. Routinely participates in program meetings, team staffing and agency meetings as required.

12. Regularly maintains monthly productivity of 123 units. 

13. Accurately completes all documentation requirements per SalusCare policy.

14. Independently manages daily schedule by prioritizing work responsibilities and individual consumer needs.

15. Routinely demonstrates good attendance and reliability by doing the following: consistently reports to work on time; reliable work performance; no abuse of leave/PTO policy; adequately plans leave and makes every effort to cover responsibilities in exigencies and coordinates training/workshops/meetings with supervisor as necessary.  

16. Utilizes staff training opportunities to enhance skills needed for delivery of patient services.  Obtains a minimum 16 hours of training per year and attends all mandatory training.

17. Promptly attains/maintains competency with agency electronic health record and uses it effectively within required time frames.

18. Consistently displays cultural competence and sensitivity and ensures services provided incorporate patient-specific cultural values.

19. Independently operates a motor vehicle in various conditions (ex: daytime, nighttime, fog, rain) and maintains a valid Florida driver’s license and auto insurance. Consistently adheres to Florida traffic and driving safety regulations.

20. Carefully adheres to peer-to-patient boundaries as per the Florida Peer Certification standards and maintain active certification status.  

21. Successfully completes other related duties as assigned by supervisor. 

Job Qualifications:
  • Requires a H.S. diploma or Equivalent. Peer Specialist Certification must be obtained within six (6) months of hire.
  • Requires lived experience with having been or currently being a recipient of substance use disorder and or co-occurring services.
  • Must possess a valid Florida driver’s license, current auto insurance, a reliable vehicle and a good driving history. 
  • Computer skills in Windows operating systems required.
  • Must be able to effectively communicate with individuals who may be confused and/or irate. 
  • Must be able to independently maneuver through a variety of environments (including those that are unclean) and interact effectively with persons from various cultural backgrounds.
