1. Routinely provides care coordination, referral services, and life skills to Transitional Living Campus patients and their families.

2. Successfully facilitates and supervises leisure activities, patient/resident meetings and other activities.  

3. Effectively interacts with patients, their family members and friends one-on-one and by phone.

4. Regularly evaluates and addresses patient needs for basic life skills education.  Facilitate life skill educational groups.

5. Reliably assists patients, on an individual and/or group basis, in processing and managing their issues/concerns. 

6. As needed, cautiously transports patients. Carefully adheres to Florida traffic and driving safety regulations during each and every transport.

7. Diligently maintains case record documentation of patient’s participation and progress in individual and group life skill and recreational sessions to meet HUD and other related contracts requirements/standards.

8. Consistently works to improve patient care and record keeping and to respond positively to peer and supervisory feedback to improve service delivery.

9. Effectively conducts screenings and assessments; group therapy and case management; discharge/transition planning and revision and crisis counseling to target population.

10. Routinely displays a working knowledge of applicable state & federal laws, CARF, SalusCare, and department policies and procedures.

11. Attains and maintains a working knowledge of multiple addiction recovery programs and patient support systems, such as 12-Steps Programs.   

12. Appropriately uses time management to ensure efficient patient care.  

13. Consistently complies with mandates of all SalusCare/DECAF/CARF and other applicable fund source regarding documentation - including quality and timeliness.  Routinely demonstrates a sound understanding of and commitment to compliance with these standards for the TLC unit.

14. Dependably works hours that are needed to ensure the needs of program are met.

15. Appropriately handles difficult situations with co-workers and patients. 

16. Routinely and reliably attends relevant meetings/committees and offers improvement suggestions. Regularly shows a willingness to assist supervisory staff in implementing new policies and procedures.

17. Steadfastly promotes an overall professional presentation to co-workers, patients, and patients collateral sources.    Conducts self in professional and ethical manner with co-workers and patients.  

18. Routinely takes on additional duties to assist supervisor and co-workers.  Dependably works hours that are needed to ensure the needs of the department are met. 

19. Maintains a consistent minimum productivity level of 1,456 hours of direct service annually.

20. Reports to supervisor any problems in program effectiveness.  Works with other staff to ensure patient needs are met.

21. Routinely demonstrates good attendance and reliability by doing the following: consistently reports to work on time; reliable work performance; no abuse of leave/PTO policy; adequately plans leave and makes every effort to cover responsibilities in exigencies and coordinates training/workshops/meetings with supervisor as necessary.  

22. Continuously utilizes staff training opportunities to enhance skills needed for delivery of patient services.  Obtains a minimum 16 hours of training per year and attends all mandatory training.  

23. Attains/maintains competency with agency electronic health record and uses it effectively within required time frames.

24. Consistently displays cultural competence and sensitivity and ensures services provided incorporate patient-specific cultural values.

25. Successfully completes other related duties as assigned by supervisor.

Job Qualifications:
Bachelor’s degree from an accredited university in a human services field required.  Requires the ability to assist patients to habilitate or rehabilitate with basic life skills and the ability to facilitate life skills, recreational and leisure groups. Bilingual (English/Spanish) preferred.  Must be able to attain competency with company electronic health record and use it effectively within required timeframes. Must possess a valid Florida driver’s license, an appropriate level of automobile insurance, a good driving history and a reliable vehicle.
