Job Responsibilities:
1. Effectively provides training, education, and support to persons with severe and persistent mental illness and co-occurring disorders in the areas of: Medication education, medication side effects, treatment compliance, metabolic disorder management/prevention, and primary care coordination.

2. Completes required medical assessments for each assigned patient in an accurate and timely manner. 

3. Productively links patients to and attends physician appointments with patients.

4. Successfully identifies and targets high risk individuals for the service.

5. Efficaciously identifies and targets care gaps.

6. Reliably provides team members with recommended treatment goals/objectives based on specific needs identified in the nursing assessment.  

7. Provides individual care planning with patients in an effective manner.

8. Successfully facilitates patient compliance with psychiatry goals/objectives.

9. Unfailingly ensures that the patient consents for all prescribed medications.

10. Appropriately monitors physical health status and adherence to treatment recommendations.

11. Meaningfully assists in the discharge planning for patients being served in a hospital setting.

12. Effectively communicates with the hospital regarding all hospital referrals.

13. Provides ongoing communication with collateral services providers.

14. Consistently demonstrates a commitment to service excellence and quality assurance and improvement.

15. Actively participates in planning committees and work groups within SalusCare system and within the community.

16. Unfailingly complies with rules/regulations/requirements related to: 1) clinical documentation, 2) all other documentation such as Outcomes, Demographics, 3) productivity, 4) Medicaid, 5) AHCA, 6) County Regulations, 7) 65E-15 and 65D-30.

17. Continuously acquires, maintains and demonstrates current knowledge and competency; through knowledge of nursing best practices.

18. Constantly demonstrates and models attitudes and knowledge consistent with co-occurring best-practice standards.

19. Consistently exhibits cultural sensitivity and ensures that staff incorporate cultural issues throughout care i.e. assessment, service planning/treatment planning, service delivery, and discharge.  

20. Reliably ensures services delivered are medically necessary and authorized for reimbursement.

21. Regularly maintains compliance with staff development training requirements.

22. Routinely exhibits a sound understanding of and commitment to complying with CARF standards.

23. Continuously demonstrates good attendance and reliability by doing the following: consistently reports to work on time; reliable work performance; no abuse of leave/PTO policy; adequately plans leave and makes every effort to cover responsibilities in exigencies and coordinates training/workshops/meetings with supervisor as necessary.  

24. Utilizes staff training opportunities to enhance skills needed for delivery of patient services.  

25. Meets CEU requirements for licensure/certification.

26. Will attain/maintain competency with agency electronic health record and use it effectively within required time frames. 

27. Successfully completes other related duties as assigned by supervisor.

Job Qualifications:
  • Requires a Florida nursing license (Licensed Practical Nurse).
  • Requires a valid FL driver’s license, a good driving record and current auto insurance.
  • Must be proficient in Windows-based computer operating system.
  • Requires ability to attain competency with the company electronic medical record and use it effectively within required time frames.
  • Bilingual and one year experience working with the mental health population preferred.
