Licensed Practical Nurse

Job Responsibilities:
1. Effectively provide health and wellness education and/or training, education, support to persons with severe and persistent mental illness, substance use and/or co-occurring disorders on medication education, medication side effects, treatment compliance, metabolic disorder management/prevention, medical, physical and overall health conditions and coordination of care with other treatment providers.   

2. Accurately and thoroughly completes required program documentation for each assigned patient. 

3. Consistently and effectively communicate identifies needs and provides appropriate linkage, consultation, or referrals internally and/or externally. 

4. Reliably provides team members with recommended treatment goals/objectives based on specific needs identified in the nursing assessment (excluding outpatient psychiatry).   

5. Effectively provides individual care planning with patients.
 
6. Successfully facilitates patient compliance with medical goals/objectives.

7. Facilitates drug screening for patients as indicated in designated program.

8. Regularly completes needed consent forms with patient.

9. Regularly completes necessary vitals.

10. As required by program area, maintain medication storage areas which may include inventory, descending log, supplies, temperature logs, etc…

11. Provides consistent ongoing communication with collateral service providers.

12. Regularly participate in staffings and/or treatment team meetings as scheduled. 

13. Dependably demonstrates a commitment to service excellence, quality assurance, and improvement.

14. Consistently complies with mandates of: CARF, FADAA, 65D-30, Medicaid, AHCA, Substance Abuse and Mental Health, County Regulations, 65E-15, SalusCare policies and procedures, and other regulatory bodies to routinely demonstrate a sound understanding of and commitment to compliance with these standards. 

15. Consistently maintains a minimum productivity level as indicated below, if applicable. 

16. Actively participates in planning committees and work groups within SalusCare system and community.

17. Acquires, maintains, and demonstrates current knowledge and competency. Maintains a thorough knowledge of nursing best practices.

18. Regularly demonstrates attitudes and knowledge consistent with co-occurring best-practice standards. Maintains a working knowledge of Recovery Oriented Systems of Care. 

19. Consistently demonstrates cultural sensitivity and ensures that cultural issues are incorporated throughout care i.e. assessment, service planning, service delivery, transition and discharge services.  

20. Ensures services delivered are medically necessary and authorized for reimbursement.

21. Maintains compliance with staff development training requirements.

22. Routinely demonstrates good attendance and reliability by doing the following: consistently reports to work on time; reliable work performance; no abuse of leave/PTO policy; adequately plans leave and makes every effort to cover responsibilities in emergencies. 

23. Utilizes staff training opportunities to enhance skills needed for delivery of patient services.  Obtains a minimum 16 hours of training per year and attends all mandatory training.  Continuously maintains compliance with staff development training requirements.

24. Maintains CEU requirements for licensure/certification. Coordinates training/workshops/meetings with supervisor as necessary.  

25. Effectively attains competence with Electronic Health Record software and uses it effectively within required time frames.

26. Reliably performs other related duties as assigned by supervisor.



Program Specific Job Duties
In addition to the above, staff in the identified program area shall:

HNHU:
1. Once a patient is assigned, conduct at least 3 weekly contacts for the first 30 days or as indicated by Care Coordination guidelines. 
2. Regularly links to and attends physician appointments with the patient.
3. Maintains annual productivity of 1,476 hours.

Transitional Living Campus and Vince Smith Center:
1. Routinely plans and conducts weekly group sessions and as needed. 
2. Reliably assists in the discharge planning for patients being served in a hospital setting. 
3. Accurately completes and reviews medication logs. 

Medication Assisted Treatment:
1. Consistently takes a proactive role in reviewing, assessing, and referring patients to qualifying SOR funded services. 
2. Effectively maintains 80% compliance to obtaining GPRA surveys at intake, 6-month follow up, discharge, 3-month post-discharge, and 6-month post-discharge of treatment, if necessary. 
3. Maintain productivity level of 65% of hours worked per month. 

Outpatient Psychiatry:
1. Regularly able to perform duties in a variety of locations/settings when required. 
2. Maintain productivity level of 65% of hours worked per month.
3. As assigned, answer OPP nursing phone line and coordinate patient medication, appointments, and/or other identified needs.

Job Qualifications:
Requires an L.P.N. certificate in nursing and a clear/active Florida LPN license. One year of experience in health, human services field is preferred. Must be able to attain competency with company electronic health record and use it effectively within required timeframes. Computer skills in Windows operating systems required. Bilingual a plus. If applicable, must possess a valid Florida driver’s license, current auto insurance, a reliable vehicle and a good driving history.
