Care Coordinator

Job Responsibilities:
1. Consistently works with patients referred by stakeholders, such as DOC, County Probation, SWFLSC, FIS, DCF and other agencies, in an effort to efficiently and seamlessly facilitate the patient’s movement through the criminal justice (Probation)/DCF and behavioral health systems. 

2. Effectively communicates with collateral contacts regularly and in accordance with contracts (Example: FIS, DCF, DOC, county probation or Florida Safety Council). 

3. Regularly provides education as needed to outpatient therapy patients and their collateral supports. This may include providing information on local resources and services which may meet patient needs. 

4. Consistently takes a proactive role in finding and developing relationships with referral sources to coordinate the patient’s experience at SalusCare in accordance with case plans and/or probation guidelines. Attends court as required. 

5. Ensures continuity of care and treatment progress by completing monthly progress reports in company electronic health record. 

6. Regularly supports both Outpatient Therapy staff and patients by assisting with the scheduling of appointments and following up on missed or cancelled appointments by reaching out to patients. 

7. Routinely plans/conducts group therapy sessions weekly or provides coverage as needed. This may include mental health/substance use related curriculums or psycho-educational approaches related to PSR (Psychosocial Rehabilitation). Groups may occur at different campuses, but are office based. 

8. Regularly ensures that group members comply with court requirements such as observed urine screens. Insures urine screen results are scanned into patient charts. 

9. Completes all documentation per agency policies and procedures. Completes documentation associated with DOC, DUI and county probation requirements. Obtains signature of qualified supervisor on Bio-psychosocial and Treatment plans as per 65-D 30 and insurance guidelines. 

10. Routinely consult with other treatment providers on a regular basis in order to monitor services and communicate needs of consumer. This would include other internal staff, as well as external providers. 

11. Maintains a minimum productivity level of 369 hours each quarter.

12. Consistently meets agency accreditation and regulating/funding source care standards. 

13. Routinely demonstrates a sound understanding of and commitment to complying with insurance, Substance Abuse Mental Health Administration, Department of Children and Families and CARF standards for the Outpatient Therapy Program. 

14. Consistently displays cultural competence and sensitivity and ensures services provided to include patient-specific cultural values. Incorporates cultural issues throughout care i.e. assessment, treatment planning, service delivery, and discharge.

15. Independently manages daily schedule and caseload by prioritizing work responsibilities as well as individual patient needs.

16. Actively participates in minimum of one time per month individual and group supervision meetings by presenting issues and ideas related to implementation of job duties and by receiving feedback related to job performance.

17. Routinely demonstrates good attendance and reliability by doing the following: consistently reports to work on time; reliable work performance; no abuse of leave/PTO policy; adequately plans leave and makes every effort to cover responsibilities in exigencies and coordinates training/workshops/meetings with supervisor as necessary.  

18. Will attain/maintain competency with company electronic health record and Microsoft Word and use them effectively within required time frames.

19. Utilizes staff training opportunities to enhance skills needed for delivery of patient services.  Obtains a minimum 16 hours of training per year and attends all mandatory training.  Meets CEU requirements for licensure/certification as applicable.

20. Able to perform duties in a variety of locations/settings when required. 

21. Other related duties as assigned by supervisor.

Job Qualifications:
Requires a Bachelor’s Degree in a human service field from an accredited college or university and one year experience working with adults with substance use, mental illnesses and co-occurring disorders.  Requires ability to attain competency with the company electronic medical record system and use it effectively within the required time frames.  Must be able to perform duties in a variety of locations and settings. Bi-lingual English-Spanish preferred, but not required.
