Job Responsibilities:
 1. Effectively provides clinical and psychosocial services to CSU and/or JARF patients, involving patients’ support systems including family, friends, case managers and outpatient treatment providers.

 2. Dependably coordinates and facilitates the development of individualized treatment plans and actively participates in interdisciplinary treatment team meetings.

 3. Provides thorough and accurate discharge planning information for initial treatment plans during interdisciplinary treatment team meetings.

 4. Routinely and professionally collaborates with external agencies and providers of service in such areas as mental health, substance abuse and developmental disabilities to implement an effective, safe and individualized discharge plan.

 5. Coordinates and facilitates safe and appropriate discharge plans for patients which include case management referrals and follow-up with outpatient appointments addressing patients identified needs.

 6. Routinely provides psychosocial education to patients and families as scheduled and as indicated in treatment plans.  Provides JARF therapy groups, individual therapy and activities to patients with addiction issues.

 7. Provides through and accurate intake screening on new admissions including obtaining appropriate collateral data.

 8. Effectively audits and insures the completion of Individualized Treatment Plans, Psychosocial Histories and Integrated Summaries within established time frames.

 9. Continuously maintains compliance with Florida Baker Act, ADM, AHCA and CARF standards.

 10. Consistently maintains compliance with staff development training.

 11. Completes all outcomes measures thoroughly and accurately.

 12. Regularly and safely assists with de-escalation of agitated patients utilizing approved TEAM & MAB techniques.

 13. Routinely demonstrates a sound understanding of and commitment to complying with CARF standards for the Crisis Stabilization Unit Programs.

 14. Consistently displays cultural competence and sensitivity and ensures services provided incorporate patient-specific cultural values.

 15. Successfully attains/maintains competency with company electronic health record and uses it effectively within required time.

 16. Reliably completes other related duties as assigned

Job Qualifications:
Requires a Bachelor’s degree in a human services field from an accredited college or university in social work, psychology, counseling or related field. 
Requires 1 year of experience providing direct care services in a behavioral health setting. 
Co-occurring experience preferred.
Must be able to attain competency with the electronic health record and use it effectively within required time frames.
