Job Responsibilities:
1. Provides clinical Intake/Assessment, Discharge Planning and psychosocial services to Acute Care Detox patients, involving patient support systems including family, collateral sources, case managers and outpatient treatment providers.
2. Coordinates and facilitates the development of initial treatment plan which is individualized and includes a medical plan for stabilization and detoxification, psychosocial education, and therapeutic activities. The treatment plan shall address how patient needs are met. 
3. Provides discharge planning information and actively participates during interdisciplinary treatment team meetings.
4. Collaborates with internal/external agencies and providers of service in such areas as housing, substance use, mental health, and development disabilities to implement an effective, safe and individualized discharge plan.
5. Coordinates and facilitates safe and appropriate discharge plan referrals which include case management, TLC, MAT Recovery Residents and follow-up with outpatient appointments addressing patient’s needs.
6. Provides psychosocial education to patients and families as scheduled and as indicated in treatment plans.  Provides psychosocial groups, individual counseling and therapeutic activities to patients with substance use disorders.
7. Insures the completion of a substance abuse assessment, initial individualized treatment plan, and psychosocial assessment within established time frames.  These assessments may identify a patient who has a mental illness, substance use, or other needs.  Those needs shall be accommodated directly or through referrals.
8. Assists in answering incoming calls, scheduling admissions, obtaining financial information and verifying insurance through Medifax, Navinet and Availity.
9. Provides intake screening on new admissions and assists with the admission process of the patient.
10. Maintains compliance with Florida Baker Act, Marchman Act, CFBHN, ADM, AHCA and CARF standards.
11. Maintains compliance with staff development training.
12. Completes all outcomes measures thoroughly and accurately.
13. Assists with de-escalation of agitated patients utilizing MAB techniques.
14. Routinely demonstrates a sound understanding of and commitment to complying with CARF standards for the Acute Care Programs.
15. Consistently displays cultural competency and sensitivity and ensures services provided incorporate patient-specific values.
16. Routinely demonstrates good attendance and reliability by doing the following: consistently reports to work on time; reliable work performance; no abuse of leave/PTO policy; adequately plans leave and makes every effort to cover responsibilities in exigencies and coordinates training/workshops/meetings with supervisor as necessary.
17. Will attain/maintain competency with agency clinical computer software and use it effectively within required time.
18. Appropriately uses time management to ensure time is used efficiently and patient care is provided in a timely and efficient manner.
19. Provides timely and quality documentation in accordance to SalusCare policies and procedures.
20. Successfully performs other related duties as assigned.
 

Job Qualifications:
Bachelor’s degree from an accredited college or university in social work, clinical psychology counseling or a related human services field.  CAP preferred. Requires a minimum of (1) one year experience providing co-occurring mental health and substance abuse services. Bilingual (English/Spanish) preferred. Must be able to attain competency with company electronic health record and use it effectively within required timeframes.
