Job Responsibilities:
•
Effectively works with individuals with a mental health and/or co-occurring substance abuse diagnosis to assist in maintaining and increasing independence through Psychosocial Rehabilitation. Also individuals identified as High Needs / High Utilizer of services within Lee County by CFBHN. Work as part of a multidisciplinary treatment team in conjunction with Case Manager to provide support to consumers in their natural environment. This service is intended to restore the individual's skills and abilities essential for independent living. This would include but is not limited to training, education, mentoring and support, based on individualized assessment of the skill deficits.
•
Efficiently provides assistance with activities which include, but are not limited to, the development and maintenance of necessary daily living skills, food planning and preparation, money management, maintenance of the living environment, and training in appropriate use of community services and resources, as well as the facilitation of cognitive and socialization skills for effective life management.
•
Routinely provides training and education on the ability to, and importance of, daily medication use, independent living and social skills training, support to individuals and their families, housing, social support and network enhancement, structured activities to diminish tendencies towards isolation and withdrawal, and teaching of the individual and family about advocating and communicating independently with providers regarding symptom management, medication and treatment options.

Effectively determines frequency of services based on need with a minimum of one monthly home visit. Makes weekly phone calls a minimum of 3x per week.

Will participate in weekly staffing's with treatment team.
•
Routinely advocates on behalf to the consumer in order to maintain needed resources; advocates in a manner that promotes independence by encouraging and teaching consumers to advocate for themselves.
•
Accurately completes treatment plan development, revision and review, as needed or required in collaboration with program participants.
•
Regularly consults with other treatment providers on a regular basis in order to monitor services and communicate needs of consumer.
•
Consistently demonstrates and models attitudes and knowledge consistent with co-occurring best-practice standards.
•
Routinely demonstrates cultural sensitivity and ensures that staff incorporate cultural issues throughout care i.e. assessment, service planning, service delivery, and discharge.
•
Accurately completes all documentation requirements per SalusCare policy.
•
Independently manages daily schedule by prioritizing work responsibilities and individual consumer needs.
•
Maintains a minimum productivity level of 1 ,472 hours of direct service annually.
•
Actively participates in minimum of once per month individual supervision meetings by presenting issues and ideas related to implementation of job duties and by receiving feedback related to job performance.
•
Consistently complies with mandates of: CARF, Medicaid, Substance Abuse and Mental Health and SalusCare policies and procedures and routinely demonstrates a sound understanding of and commitment to compliance with these standards.
•
Clearly demonstrates an understanding of and thoroughly adheres to the Florida Medicaid Handbook related to Community Support and Rehabilitative Services/Psychosocial Rehabilitation Services, including, but not limited to, the restrictions, documentation requirements and reimbursable activities.
•
Effectively attains competence with company electronic medical record and uses it successfully within required time frames.
•
Maintains compliance with Staff Development training requirements.
•
Independently manages daily schedule by prioritizing work responsibilities and individual consumer needs.
•
Successfully performs other related duties as assigned by supervisor.
Job Qualifications:
Bachelor Degree in a human service field from an accredited college or university. Must possess a valid Florida driver's license and the appropriate level of automobile insurance coverage. Must have a reliable vehicle. Computer skills in Windows operating systems required.
