Job Responsibilities:
The Case Manager position requires consistent exercise of discretion and judgment within the context of Saluscare policies/procedures, related training, and supervision.

1. Efficiently works with mental health consumers in an effort to effectively link them to needed resources, coordinate such resources as necessary, and monitor the implementation of such.  Examples of such resources may include, but are not limited to, psychiatric services, rehabilitation services, Supplemental Security Income (SSI), Medicaid, Patient Assistance Programs, family members, landlords, and employers.  Links and coordinates resources in a manner that promotes consumer independence.

2. Routinely advocates on behalf of the consumer in order to obtain and maintain needed resources; advocates in a manner that promotes consumer independence by encouraging and teaching consumers to advocate for themselves.

3. Consistently takes a proactive role in finding and developing community-based resources in order to offer consumers a wider array of choices with regard to respective needs i.e. identifying housing options, employers, medical and mental health professionals.

4. Routinely consult with other treatment providers on a regular basis in order to monitor services and communicate needs of consumer.

5. Effectively demonstrates and models attitudes and knowledge consistent with co-occurring best-practice standards.

6. Effectively demonstrates cultural sensitivity and ensures that staff incorporate cultural issues throughout care i.e. assessment, service planning, service delivery, and discharge.  Provide services that respect the varied identities people have based on race, ethnicity, age, sexual orientation, gender and gender identification, religion, spirituality, and mental, emotional and physical differences in abilities.

7. Regularly provides education as needed to mental health consumers and their collateral supports. 

8. Consistently and accurately completes assigned assessments and service plans with each person served within his or her natural environment, in accordance with Saluscare program policies and procedures. 

9. Routinely conducts minimum of monthly contact (minimum of every other month home visits) in order to provide support and conduct ongoing assessment of needs and monitoring of service plan needs.

10. Efficiently completes all documentation requirements per Saluscare policy.

11. Effectively serves a maximum of 40 individuals at a given time.

12. Independently manages daily schedule by prioritizing work responsibilities and individual consumer needs.

13. Consistently maintains a minimum productivity level of 1,472 hours of direct service annually. 

14. Actively participates in minimum of twice per month individual supervision meetings by presenting issues and ideas related to implementation of job duties and by receiving feedback related to job performance.

15. Consistently complies with mandates of: CARF, Federal Court Consent Decree, FAC 65-E15, DCF Contract, complies with Saluscare and Florida Certification Board Code of Ethics and Saluscare policies and procedures and routinely demonstrates a sound understanding of and commitment to compliance with these standards for the Diversion Program. 

16. Consistently protect the client’s right to privacy and confidentiality according to laws and other governing rules when preparing and handling records, especially regarding the communication of client information with third-parties. 
a. Follow federal, state and other governing rules and regulations in order to promote and protect client rights.
17. Clearly and effectively demonstrates an understanding of and thoroughly adheres to the Mental Health Targeted Case Management Handbook including, but not limited to, the restrictions, documentation requirements and reimbursable activities. 

18. Effectively links consumers to independent means of transportation; provides limited transportation to adult consumers as needed and indicated in service plan if independent transportation has not yet been secured.

19. If designated, will effectively be a liaison between SalusCare and State Mental Health Treatment Facilities (i.e. South Florida State Hospital) to participate in monthly telephone client treatment team staffings, bi-monthly face to face visits and to develop discharge plan 

20. Accurately operate a reliable motor vehicle during daytime and nighttime driving, and maintain a valid Florida driver’s license.

21. Consistently maintains competence with company electronic medical record and uses it effectively within required time frames.

22. Effectively demonstrate ability to perform duties in a variety of locations/settings.

23. Consistently maintain compliance with Staff Development training requirements.

24. Routinely performs other related duties as assigned by supervisor.


Job Qualifications:
Bachelor’s degree from an accredited university or college with a major in counseling, social work, psychology, criminal justice, nursing, rehabilitation, special education, health education, or a related human services field (a related human services field is one in which major course work includes the study of human behavior and development)
And one year of full time or equivalent experience working with adults experiencing serious mental illness.
Must possess a valid Florida driver’s license and the appropriate level of automobile insurance coverage. Must have a reliable vehicle.
Computer skills in Windows operating systems required.

Training Requirements: 
Complete AHCA-approved mental health targeted case management training within three months of initially providing Medicaid services.
Within 1 year must obtain Certified Behavioral Health Case Manager Certification from Florida Certification Board.
