Licensed Practical Nurse

Job Responsibilities:
1. Reliably assists with check-in of clients. Reviews, updates and maintains client records.

2. Consistently completes vital signs on all clients and enters into the electronic medical record.

3. Regularly prepares treatment plans, explains to clients and obtains client signature.

4. Consistently obtains client consent, explains to client and obtains client signature.

5. Accurately assists with scheduling appointments as necessary.

6. Regularly completes telephone triage, retrieves messages, obtains charts and returns phone calls within 24 hours.

7. Obtains insurance authorizations for client medication and ensures completion of processes and communication with client acting as back-up for phone nurse. 

8. Professionally interacts and coordinates with pharmacies, hospitals, community physicians, and laboratories.

9. Professionally and respectfully conducts his/her self in a manner defined by SalusCare Ethical Code of Conduct as it pertains to providers, managers, co-workers, patients and other stakeholders.

10. Reliably provides medication information and obtains consents as necessary.

11. Administers injections as prescribed by Physician or ARNP and documents appropriately in Electronic Medical Record.  

12. Effectively and efficiently supports multiple providers throughout the day providing client work-up, monitoring his/her schedule to maximize patient volume and assuring all available appointment times are filled. 

13. Productively works with support staff to manage walk-in list at all locations, filling provider schedule for maximum efficiency and productivity 

14. Carefully reviews client record with Physicians or ARNP when necessary.

15. Maintains appropriate customer service relationship and professional boundaries at all times.

16. Routinely checks medications in med room for expiration date and appropriately discards and notes discard of outdated medications.

17. Attends regularly scheduled department meetings as requested by supervisor.

18. Attends and meaningfully participates in other agency meetings as requested by the supervisor.

19. Routinely demonstrates a sound understanding of and commitment to complying with CARF standards for the Outpatient Department.

20. Consistently displays cultural competence and sensitivity and ensures services provided (evaluations, treatment plans, discharge planning) incorporate client-specific cultural values.

21. Successfully attains competence with Electronic medical record software and use it effectively within required time frame.

22. Maintains active participation in at least one Committee or Performance Improvement Team (PIT) and attends that and other meetings as assigned or necessary.

23. Thoroughly and accurately completes chart peer reviews on a monthly basis and follows up on any deficiencies noted.

24. Consistently demonstrates and models attitudes and knowledge consistent with co-occurring best-practice standards and ensures staff are trained and practice accordingly.

25. Continuously demonstrates cultural sensitivity and ensures that staff incorporate cultural issues throughout care i.e. assessment, service planning, service delivery, and discharge.

26. Operates as a back-up to alternate department(s) when requested.

27. Maintains compliance with staff development training. 

28. Successfully performs other related duties as assigned by supervisors.

Job Qualifications:
Requires an L.P.N. certificate in nursing
A clear/active Florida LPN license.
One year of experience in health, human services field is preferred.
Must be able to attain competency with company electronic health record and use it effectively within required time frames.
Requires basic clerical skills including but not limited to writing, taking accurate phone messages, working with multiline phone system, copying, faxing, shredding, basic computer literacy.
