Job Responsibilities:
The Children’s Case Manager position requires consistent exercise of discretion and judgment within the context of SalusCare policies, procedures, related training, and supervision.  

1. Effectively provides case management services to children and youth in need of coordinated community based mental health services in order to avert more restrictive out of home residential placement.

2. Competently maintains a maximum caseload of 20 patients and produces minimal productivity level of 1472 hours annually.

3. Consistently complies with all statutory regulations that affect the target population.  Follow established professional code of ethics and standards of practice, for both SalusCare and Florida Certification Board, in order to promote the best interests of the patient and the profession.

4. Consistently maintains case record documentation to meet DCF, Medicaid, and SalusCare policies and procedures. 

5. At all times will protect the client’s right to privacy and confidentiality according to laws and other governing rules when preparing and handling records, especially regarding the communication of client information with third-parties.  Obtain written consent to release information from the client and/or legal guardian according to agency protocols and administrative rules. 

6. Efficiently conduct activities designed to procure specified services, treatment, and resources for the patient by means of telephone calls, meetings, verbal and written referrals, assistance with making appointments and completing applications and assistance with interview and hearings.

7. Accurately administer the appropriate assessment instruments to gather information and initiate a process of shared decision making and goal setting focused on patient strengths, needs, abilities and preferences. With the patient, develop a written service plan that reflects assessment outcomes.

8. Routinely monitor activities, including contact with the client, concerned others, and service providers to assess the effectiveness of the case plan strategies and measure progress on achieving case plan outcomes, as well as identify emerging needs.

9. Effectively assume an advocacy role for patients to assure access to services documented in the service plan and monitor that these services are provided in a timely and appropriate manner. Monitor progress or lack of progress toward goals and make changes as necessary. This includes arranging case staffings as necessary.

10. Consistently demonstrates and models attitudes and knowledge consistent with co-occurring best-practice standards.  Seek supervision, training and technical assistance as necessary to maintain competence and enhance professional effectiveness.

11. At all times demonstrates cultural sensitivity and incorporate cultural issues throughout care i.e. assessment, service planning, service delivery, and discharge.  Provide services that respect the varied identities people have based on race, ethnicity, age, sexual orientation, gender and gender identification, religion, spirituality, and mental, emotional and physical differences in abilities.

12. Routinely develop professional networks in order to identify and maintain information about available community resources to meet client needs.

13. Routinely attend staff meetings, in-service trainings, supervision, and multi-disciplinary inter-agency meetings as required.

14. Appropriately terminate case management services, to include providing reasonable notice; securing client and/or significant other’s acknowledgement of, and agreement to, the case termination; and, if appropriate, effectively and efficiently transitioning the client to the next level of care.  Arrange transitional services for all clients leaving our service area or are nearing their 18th birthday.  

15. Consistently maintain compliance with staff development training as per SalusCare, Medicaid, and Department of Children & Families.

16. Accurately must be able to operate a motor vehicle during daytime and nighttime driving and maintain a valid Florida driver’s license and be able to operate a standard motor vehicle restraint system (seat belt).

17. Routinely demonstrates a sound understanding of and commitment to complying with CARF standards for the Children’s/ Case Management Program.

18. Effectively attains competence with company electronic medical record and uses it effectively within required time frames.

19. Successfully performs other related duties as assigned by supervisor.

Job Qualifications:
Bachelor’s degree from an accredited university or college with a major in counseling, social work, psychology, criminal justice, nursing, rehabilitation, special education, health education, or a related human services field (a related human services field is one in which major course work includes the study of human behavior and development) and (1) one year of full time or equivalent experience working with children with serious emotional disturbances or have a Bachelor’s degree from an accredited university or college and (3) three years full time experience working with children with serious emotional disturbances Must possess a valid Florida driver’s license and the appropriate level of automobile insurance coverage. Must have a reliable vehicle. Must be able to attain competency with company electronic health record and use it effectively within the required timeframes.                                                                   

Training Requirements:  Completed or agrees to complete AHCA-approved mental health targeted case management training within (3) three months of initially providing Medicaid services. Within (6) six months must obtain Certified Behavioral Health Case Manager Certification from Florida Certification Board.
