Job Responsibilities:
Successfully works with patients in an effort to effectively link them to needed resources, coordinate such resources as necessary, and monitor the implementation of such. Examples of resources may include, but are not limited to, recovery-oriented support systems, mental health resources, substance use services and treatment, housing, financial, vocational, connection to social activities, and other resources as needed to promote independence and enhance daily living.
Routinely advocates on behalf of the patient in order to obtain and maintain needed resources; advocates in a manner that promotes patient independence by encouraging and teaching patients to advocate for themselves.
Consistently takes a proactive role in finding and developing a listing of community-based resources in order to offer patients a wider array of choices with regard to respective needs i.e. identifying housing options, employers, medical and substance abuse/mental health professionals.
Works closely with the SalusCare milieu of services, local hospitals, physician groups, and other community agencies to collaborate, facilitate, and advocate for services as needed for patients.
Routinely consults with other treatment providers on a regular basis in order to monitor services and communicate needs of patient.
Consistently work to engage individuals into treatment and services.
Routinely provides education and/or educational materials to the patient and/or their collateral supports as related to presenting treatment needs.
Promptly and accurately completes required documentation for each person served in accordance with SalusCare policies and procedures. This shall include, but not limited to, screenings, assessments, and discharge planning documentation.
Conduct required number of monthly contacts as specified in the identified program listed below (if applicable), in order to provide support and conduct ongoing assessment of needs and monitoring of treatment plan needs, to also include needed crisis interventions.
Regularly prepare and compile needed patient data to link to internal and external partners, as needed.
Independently manages daily schedule by prioritizing work responsibilities and individual patient needs.
Maintains a minimum productivity level of 123 hours per month.
Effectively coordinates with other programs on mutual patients to align treatment, as needed.
Actively participates in supervision meetings with primary supervisor by presenting issues and ideas related to implementation of job duties and by receiving feedback related to job performance.
Consistently complies with mandates of: CARF, FADAA, 65D-30, Medicaid, AHCA, Substance Abuse and Mental Health, SalusCare policies and procedures, and other regulatory bodies to routinely demonstrate a sound understanding of and commitment to compliance with these standards.
Routinely demonstrates good attendance and reliability by doing the following: consistently reports to work on time; reliable work performance; no abuse of leave/PTO policy; adequately plans leave and makes every effort to cover responsibilities in exigencies and coordinates training/workshops/meetings with supervisor as necessary.
Consistently participate in discharge planning, treatment team meetings, conference calls, and other identified external coordination of care meetings. Attend program and department meetings as required.
Regularly demonstrates and models attitudes and knowledge consistent with co-occurring best-practice standards. Maintains a working knowledge of Recovery Oriented Systems of Care.
Consistently demonstrates cultural sensitivity and ensures that cultural issues are incorporated throughout care i.e. assessment, service planning, service delivery, transition and discharge services.
Effectively attains competence with Electronic Health Record software and uses it effectively within required time frames.
Utilizes staff training opportunities to enhance skills needed for delivery of patient services. Obtains a minimum 16 hours of training per year and attends all mandatory training. Continuously maintains compliance with staff development training requirements.
Reliably performs other related duties as assigned by supervisor.
Program Specific Job Duties
In addition to the above, staff in the identified program area shall:
Outpatient Therapy:
1. Routinely plans and conducts weekly group sessions and as needed.
2. Regularly able to perform duties in a variety of locations/settings when required.
3. Consistently monitor and implement the Pathway Forward initiatives.
Transitional Living Campus:
1. Routinely plans and conducts weekly group sessions and as needed.
2. Conduct at least 1 weekly face to face contact with assigned patient caseload.
Medication Assisted Treatment:
1. Consistently takes a proactive role in reviewing, assessing, and referring patients to qualifying SOR funded services.
2. Effectively maintains 80% compliance to obtaining GPRA surveys at intake, 6-month follow up, discharge, 3-month post-discharge, and 6-month post-discharge of treatment.
3. Conduct at least monthly contacts with patient caseload.
Outpatient Psychiatry:
1. Regularly able to perform duties in a variety of locations/settings when required.
HNHU:
1. Once a patient is assigned, conduct at least 3 weekly contacts for the first 30 days or as indicated by Care Coordination guidelines.
Job Qualifications:
Bachelor’s degree from an accredited university or college with a major in counseling, social work, psychology, criminal justice, nursing, rehabilitation, special education, health education, or a related human services field (a related human services field is one in which major course work includes the study of human behavior and development). Computer skills in Windows operating systems required. Must be able to attain competence with company electronic health record.
