Job Responsibilities:
1. Effectively provides clinical and psychosocial services to CSU patients, involving patients’ support systems including family, friends, case managers and outpatient treatment providers.

2. Dependably coordinates and facilitates the development of individualized care plans and actively participates in interdisciplinary treatment team meetings.

3. Provides thorough and accurate discharge planning information for initial care plans during interdisciplinary treatment team meetings, and offer potential resolutions such as Assisted Outpatient Treatment. 

4. Ensure patients receive up-to-date resources in the community as a part of their discharge planning process. 

5. Routinely and professionally collaborates with external agencies and providers of service in such areas as mental health, substance use and developmental disabilities to implement an effective, safe and individualized discharge plan.

6. Coordinates and facilitates safe and appropriate discharge plans for patients which include case management referrals and follow-up with outpatient appointments addressing patients identified needs.

7. Routinely provides psychosocial education to patients and families as scheduled and as indicated in treatment plans.  Provides needed therapy groups, individual therapy and activities to patients with substance use issues, if needed.

8. Provides through and accurate intake screening on new admissions including obtaining appropriate collateral data.

9. Effectively audits and insures the completion of care plans, comprehensive assessments, and summaries within established time frames.

10. Continuously maintains compliance with Florida Baker Act, DCF, AHCA and CARF standards.

11. Consistently maintains compliance with staff development training.

12. Completes all outcomes measures (CFARS, FARS, and Outcomes) thoroughly and accurately.

13. Ensures documentation is completed and signed within the agencies timeliness requirement, and is completed accurately.

14. Regularly and safely assists with de-escalation of agitated patients utilizing approved TEAM.

15. Routinely demonstrates a sound understanding of and commitment to complying with CARF standards for the Crisis Stabilization Unit Programs.

16. Consistently displays cultural competence and sensitivity and ensures services provided incorporate patient-specific cultural values.

17. Successfully attains/maintains competency with company electronic health record and uses it effectively within required time.

18. All discharge planners working at the CSU are qualified and may be asked to work as a discharge planner either on the adult or child CSU. 

19. Reliably completes other related duties as assigned.

Job Qualifications:
Requires a Bachelor’s degree in a human services field from an accredited college or university in social work, psychology, counseling or related field.  Requires 1 year of experience providing direct care services in a behavioral health setting.  Co-occurring experience preferred. Must be able to attain competency with the electronic health record and use it effectively within required timeframes.
