Job Responsibilities:
Provide therapy services for a diverse population of patients with either mental health or substance abuse related diagnoses utilizing best practice and evidence based modalities.  This may include children, families and adult services. Services are office based, but may be provided out of various locations underneath the outpatient therapy program. 

1. Effectively provides individual, group, and family therapy as scheduled and as indicated in the treatment plan/program plan. 

2. Consistently uses individual, group and family therapy practices that are research and evidence based. This also includes assessment and evaluation of children, adults, and older adults with substance abuse, mental health and/or co-occurring disorder.  Will also be able to use sound judgment when referring to the appropriate providers and treatment needed.  

3. Routinely engages in peer review.  Minimum of two charts reviewed per clinician, per month.

4. Promptly attends to any deficiencies in paperwork as identified through audits or quality review.  Complete outcome measures timely and accurately.  

5. Successfully develops treatment plan collaboratively with clients that are strength based and use person centered language. Ensures that supervisor signs off on treatment plans as per insurance and 65-D 30 guidelines. Involve family/collaterals in treatment planning and ongoing treatment, as appropriate.

6. Satisfactorily conducts treatment plan reviews with client as needed, or minimally every 90 days. Addresses co-occurring disorders on treatment plan. Obtains supervisor signature in compliance with insurance and 65D-30 guidelines. 

7. Reliably deliver quality services to patients in person and/or via the currently approved platform by which tele services will be offered to patients of SalusCare.

8. Reliably complete appropriate referrals to other providers for patients with disorders that are contraindicated for tele services. 
9. Routinely provides excellent customer service as evidenced by being on time for appointments, returning phone calls within 24 hours and 90 percent satisfaction on client satisfaction surveys.

10. Consistently adheres to standards of clinical documentation, client rights, and confidentiality. 

11. Accurately completes paperwork in accordance with SalusCare agency policies and procedures, contract agencies, DCF, CARF, 65D 30 standards and monitoring groups.

12. Reliably coordinates care including making referrals as needed and helping patients access case management services as needed.

13. Routinely provides support in facilitating psychiatric services and Baker Act procedures as appropriate.

14. Consistently meets productivity expectations which includes 67% of time face to face.   

15. Regularly participate in staff development activities.

16. Routinely demonstrates good attendance and reliability by doing the following: consistently reports to work on time; reliable work performance; no abuse of leave/PTO policy; adequately plans leave and makes every effort to cover responsibilities in exigencies and coordinates training/workshops/meetings with supervisor as necessary.  

17. Effectively attain competence with agency electronic health record and use it effectively within required time frames.

18. Consistently displays cultural competence and sensitivity and ensures services provided incorporate client – specific cultural values. 

19. Routinely demonstrates a sound understanding of and commitment to complying with CARF standards for the Outpatient Therapy Program.

20. Continuously maintains compliance with staff development training curriculum requirements.

21. Successfully performs other related duties as assigned by supervisor.
 

Job Qualifications:
Must have a Master’s degree or higher from an accredited college or university in a human services field with a major in counseling, social work, psychology or related human services field.  A minimum of (1) one year of experience working with individuals with mental health, substance abuse and co-occurring disorders.  Must be comfortable working with children, families and adults in multiple modalities and locations.  Applicants should also be comfortable assessing and/or working with children, families and adults. Requires the ability to perform duties in a variety of locations and settings as you may be required to travel to various office locations at Cape, Evans, Lehigh, or Ortiz. Must be able to attain competency with company electronic health record and use it effectively with required timeframes.
